Step-by-Step Guide on How to Apply for Home Care through the HSE
- Contact Your Local Health Centre
Begin by contacting your local HSE Health Centre and requesting a visit from a
public health nurse. If you’re eligible for home support, the nurse will provide you
with the necessary application form to complete.
- Complete the Application Form
You can fill out the application form yourself, or you can ask someone to assist you.
It’s important that you, as the person needing the service, sign the Declaration and
Consent section in Part 1 of the form. Only someone with legal authority can sign Part
1 on your behalf.
- Assisted Decision Making
Under the Assisted Decision Making (Capacity) Act 2015, which came into effect on
26th April 2023, you have the option to nominate someone to assist you in making
decisions about the application. If you choose to have a decision supporter, they must
explain why they are completing the form for you in Part 2. Your public health nurse
will be available to provide further details on this.
- Return the Completed Application
Once you’ve completed the application form, the public health nurse will submit it on
your behalf. If you need assistance with filling out the form or have any questions, the
nurse will be happy to help.
- If You Are in Hospital
If you’re currently in the hospital and need to apply for home care, ask the person
responsible for your discharge plan to provide you with a Home Support Application
Form. It’s advisable to request this as early as possible during your hospital stay to
avoid delays when you’re ready to return home.
